As a Clinical Pharmacist, you will maintain or enhance clinical pharmacy programs such as health outcomes, disease state management, care management, and other speciality programs.

Position Duties and Responsibilities:
Performing in home setting pharmacotherapy consultations with patients and/or prescribers promoting high quality, evidence-based, cost-effective pharmaceutical care
Assess the patient’s response to drug therapy and planning drug therapy based on physician-established diagnoses.
Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.
Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff.
Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly.
Consult with other health care providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.
Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients.
Assessing patients for medication adherence barriers and developing a plan to maximize patients' medication success through improving or maintaining adherence to medications
Developing a plan of care that details actions of the patient and/or prescriber based on the pharmacotherapy assessment
Delivering high quality evidence-based therapeutic recommendations to providers
Documentation of clinical consultations including the plan of care into an internal database
Participating in and contributing to staff meetings and other staff development opportunities and interdepartmental workgroups
Identifying areas of improvement where systems modifications could result in operational advancements and efficiencies
Minimum of one (1) year of experience as a Clinical Pharmacist.
Doctor of Pharmacy (Pharm.D.).
Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
Current, full, active, and unrestricted license to practice as a pharmacist.
Must be a US Citizen/authorized to work in the US.
