Full Job Description
Home Helpers of Santa Clara Valley is currently seeking for a Client Care Coordinator.
Perform a quality assurance visit for current clients and caregivers at least once/month.
Upload client information into Clear Care
Document and enter: client visit, findings, issues, and concerns into Clear Care Software.
Work with Scheduler about caregiver/client schedules
Follow up on Incident Reports Within 24 hours within the required guidelines
Ensure Client Notebook/Folder with the Minimum of Plan of Care and Schedule is in each client’s file
Schedule meeting with caregiver to review performance
Attend daily calls, health fairs, job fairs, etc. as requested
Assist with organizing meetings, in-services and gatherings as needed
Conduct home consultations
Develop Plan of Care from the required software
Enter new clients and match with caregivers in the required software
Ensure all clients have their appropriate caregiver at their scheduled time
Call client after first shift of new caregiver
Call new caregiver after first shift worked and end of first week
Assist with caregiver hiring and orientations
Create Monthly Client Newsletter

Experience with Clear Care and Microsoft Office
Prior experience in a healthcare or in-home care environment is a MUST.
Valid driver’s license and reliable transportation every single day
Willingness to enforce health and safety standards
High School Diploma or equivalent
