The position is responsible for supervising the personal care services provided to the participants by the program aides, conducting an initial assessment and subsequent reassessments of each participant for nursing and personal care services under the direction of the Nursing Supervisor. The Part-time Registered Nurse is a supporting member of the Center’s multidisciplinary health care team and shall be knowledgeable of the physical, mental, and social needs of the participants served with knowledge of clinical practices and demonstrated competence in working with adults and seniors with memory impairment, Parkinson’s disease, Stroke, and other chronic conditions.

- The Registered Nurse (RN) is licensed as such by the California Board of Registered Nurses.
- Minimum two years’ experience working with geriatric and disabled populations preferred but not required. Experience with Alzheimer’s and other related dementias is preferred.
- Possess strong computer skills and functional knowledge of the Microsoft Office Suite (Word, Excel).
- Copies of current and applicable degree(s), license(s), certification(s), liability insurance, First Aid, and Healthcare Provider CPR have been provided to the Center.
- Registered Nurse shall be required to adhere to Covid-19/Influenza practices as advised by the organization, CDC and CDA updates. This shall include the use of a N95 or surgical mask while working at the Center when appropriate and as needed.
1. Aspires to provide Person-Centered care theories and practice as required by CDA/CBAS with the understanding that a care plan shall reflect the individual’s strengths and preferences, reflect clinical and support needs as identified through an assessment of functional need, and include individually identified goals and desired outcomes.
2. Adheres to all licensing, regulation, and Center policies and procedures.
3. Coordinates necessary communication to support telehealth/virtual and other alternative service delivery methods including telephone, Facetime, Zoom when a participant is utilizing Emergency Remote Services.
4. Assist/support Lead RN in managing C.N.A/Program Aide supervision/performance evaluation and schedules, including replacement coverage.
5. Conducts in-person/telehealth assessment and creates an Individual Plan of Care for each participant: Provide all nursing care specified in the participant Individual Plan of Care. -Update and revise a participant Individual Plan of Care according to reauthorization date or participant medical change of condition. - Participate in monthly/daily MDT meetings in the absence of the Lead RN.
6. Maintain the nursing component of the participant health record which includes:
· Documentation of all nursing care provided on a day of care.
· Update progress notes/ flowsheet entries/ quarterly notes/ incident reports as needed.
· Record participant medication administration and physician ordered changes in medication regime.
· Coordinate service with Nutritionist for dietary counseling, and/or participant care.
· Monitor Federal Food Program guidelines and required documentation.
7. Provide direct nursing services to participant as needed on a day of care:
- Observation and monitoring of the participants’ health status.
- Perform participant blood pressure/ blood sugar checks, weights, incontinence care, wound care, emergency care as directed.
- Administer and/or monitor participants’ medication.
- Transfer/Lift participants on/off therapy equipment, mobility aides as needed.
- Use proper body mechanics at all times with personal care assistance, maintenance therapies, escorting, activities and other job functions.
- Supervise CNA and Program Aides in delivering personal care services to the participants.
- Supervise CNA and Program Aides in delivering maintenance therapies when PT is not present during program.
- Liaison with the participants’ physicians and notify them of any changes in the participants’ health status.
Assisting participants in obtaining medical services from providers other than the adult day health center staff.
