The social work for Children with Special Healthcare Needs provides services to patients and families with children who have physical and cognitive challenges due to serious complex medical diagnoses. As a member of the interdisciplinary team the social worker will provide assessment of patient and family, support and link them to community resources and other referral sources through advocacy, concrete services, counseling, and networking with school personnel and support. The Social Worker will be active in the community in order to assist and advocate for these patients. The worker will be available to attend and assist in Individual Education Plan (IEP) home visits and center-based programs to collaborate with community support services. Participation in Pratice Intake coverage system is required.
Skills & Abilities:
Knowledge of developmental stages of children and adults. This position interacts with patients from all ages from Neonates to Adults.
Training & Education:
Masters Degree from an accredited school of Social Work. This position interacts with patients from all ages from Neonates to Adults. Knowledge of community resources and expertise in understanding IEP,
mental health and early intervention. Knowledge of basic computer skills is an asset.
Minimum five years' graduate experience under MSW supervision.
