Perform responsible and highly complex professional duties to provide a variety of services to eligible customers to improve their ability to function in society. Duties involve direct intervention with individuals with complex problems in one or more areas such family services, adoption, adult services, and child welfare services. May act as a team leader.
EXAMPLES OF DUTIES
- Meet with customers and assess needs and current strengths and abilities. Interview customers and their families, if needed, to gather information on their social, health, emotional, and economic conditions. Make extensive use of home visits. Independently determine appropriate service delivery response to customer's need.
- Work with customers to formulate individual service plans including goals and objectives. Identify actions needed to resolve personal problems such as those related to emotional needs, finances, home management, child care and development, housing, health, education, and employment.
- Refer customers to other community agencies such as clinics, employment services, and vocational rehabilitation services. Provide direct intervention or contracts for appropriate service delivery as needed. Arrange for appropriate purchase and monitoring of purchased services. Ensure proper documentation for payments.
- Work with customers to implement service plans. Provide training for customers in a variety of areas such as financial management, coping skills, adoptive or foster parenting, etc.
- Provide protection to abandoned, abused, or neglected children and arrange for foster care as needed. Recruit, assess, and develop homes for adoption, child and adult foster care, and day care programs if needed. May conduct independent adoption investigations to determine the suitability of petitioners to adopt.
- Participate in court proceedings when necessary regarding child protective services, child custody, adoption, foster care, and adult protective services.
- Develop and maintain manual and computerized records, files, and documentation systems in all aspects of work. Utilize computer systems to update, inquire, and retrieve data for individual cases and compile reports.
- May be assigned to the Family Assessment and Planning Team (FAPT). Participates in FAPT meetings and assists with the development of Individualized Family Service Plan(s) in addition to managing a reduced caseload.
- May act as a team leader for several workers and provide training and mentoring to less experienced staff members while carrying a caseload. May direct and monitor the day to day work of other workers and/or volunteers.
- Respond effectively to customer concerns and complaints.
- Maintain current knowledge of policy, laws, and current trends and developments in the social work field. Maintain knowledge of current social and economic conditions and environment. Maintain current knowledge of available internal and external resources.
- Provide oversight and develop policies and procedures for best practice initiatives (e.g. concurrent planning, OASIS record keeping, adoption specialty services).
- Monitor adherence to the State Program Improvement Plan.
- Participate in a wide variety of meetings, training, and professional conferences. May represent the agency at community meetings and inter agency coalitions.
- Perform other related duties as assigned.
Requires graduation from an accredited college or university with a Bachelor's degree in Social Work, Sociology, Human Services, or a closely related field of study. Four years experience as a Social Worker or equivalent experience. Extensive knowledge of: current social work principles; human behavior and adult learning principles. Ability to: work independently; communicate effectively orally and in writing, especially in times of stress; make sound judgments based on knowledge of human behavior; current local, state, and federal laws, and current policy; interpret and assess individuals' and/or families' strengths and needs; organize and prioritize work; plan and manage service delivery; ensure follow-through; respond to customers effectively, especially in times of stress. Individuals under consideration will be subject to a criminal background check and child protective services check.
SUPPLEMENTAL INFORMATION Perform duties under the general supervision of assigned Family Services Supervisor. Incumbent must be able to work independently with little direct supervision and to exercise sound judgment in stressful situations. May require working beyond a standard 40-hour work week to include evenings, weekends and holidays. Requires considerable travel within the City of Hampton. This position may be considered essential/emergency personnel for purposes of emergency/crisis management to include being "on-call" or responding to emergency situations during inclement weather and/or other job-related conditions. This position is nonexempt.
