Job Title: Discharge Planner
Responsible for coordinating and managing the discharge plan for patient populations with acute and/or chronic illness or those with multiple comorbidities in the across hospital settings. The Discharge Planner works in collaboration with case managers, social workers, primary care physicians/specialists, internal and external stakeholders including but not limited to: acute/subacute rehabilitation centers, long-term acute care hospitals (LTACH), infusion and home care agencies. Performs functions related to the day-to-day operations related to care management focused on effectively and efficiently assisting patients through smooth, safe and quality transitions in care settings.
Implement the discharge assessment for all patients.
Subacute/Acute Rehabilitation, LTACH or home care Authorization process for patients with commercial or managed care.
Multi-disciplinary care round to effectuate the discharge care plan.
Prevent financial loss to hospital by effectuation of the discharge plan within the appropriate length of stay based on industry standards.
Utilize community resources to meet the discharge needs of individual patient needs.
Ensures continuity of care for patients with complex discharge needs.
Performs other duties based on departmental needs as assigned by supervisor/director.
Graduate of an accredited school of Social Work.
Licensed Social Worker in the state of New Jersey
Registered Nurse licensed in the state of New Jersey
Excellent Communication and interpersonal skills
Minimum of three (3) years of Discharge Planning experience.
Social Work or Case Management experience in the acute care medical/surgical setting.
Or a combination of education and experience.
