Under the supervision of the BCW (Babies Can’t Wait) Manager, assess client’s birth to three with identified or suspected developmental delays. Provide patient education, linkage to care and developmental assessments. Maintain confidentiality and precise data entry, following all rules and policies. Must be willing to train others. Adheres to HIPAA regulations. Facilitates the development, review and evaluation of the Individual Family Service Plan per program standards. Maintains strict timelines for implementation of services and enters all information into BIBS/VHN. This position works to promote and protect the health and safety of the residents of Cobb & Douglas Counties.
Minimum Qualifications: Bachelor's degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate's degree in nursing from an accredited college or university AND One year of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.
Preferred Qualifications: Bachelor’s degree, Public Health or Community Health experience/knowledge. Experience with Babies Can’t Wait or Early Intervention programs. Bilingual English/Spanish.
Salary Information: $26 per hour / up to 29 hours per week (No Benefits)
