We have an immediate opening for a Waiver Case Manager who can coordinate, implement, and develop high-quality services for our community's most vulnerable.

Qualifications and Requirements:
- Bachelor's Degree in health or related field preferred
- 1 year of home health or home care experience preferred
- Ability to multitask and deliver exceptional customer service in a fast-paced environment
- Ability to coordinate with various team members to achieve organizational outcomes required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
Compensation: 21-24
