Brigham and Women’s Advanced Primary Care Associates on South Huntington in Jamaica Plain is a primary care practice for adults 18 and older. A NCQA level 3 certified medical home, South Huntington is winner of the 2014 Society of General Internal Medicine Quality and Practice Innovation Award and was one of the practices profiled in the ABIM Foundation’s “In Search of Joy in Practice: A Report of 23 High- Functioning Primary Care Practices”. Our multi-disciplinary team-based approach is setting the standard for primary care in Boston.
The practice is divided into 3 clinical teams, each team including several physicians (many part time with either leadership, research or entrepreneurial interests in primary care transformation), a physician assistant, two medical assistants, 1 LPN, an RN and a social worker. Additional shared resources include a dietician, pharmacist, a community resource specialist/behavioral health population manager, a community health worker, a practice coordinator, practice manager, 2 phlebotomists, and a security officer. We strongly believe it takes a team as robust as this to meet all the needs of the diverse population we are privileged to serve. Our patients are viewed as a critical member of the team, with an active Patient Family Advisory Council.
The Primary Care Practice Medical Director is responsible for the oversight of all aspects of patient care within the practice. S/he oversees the clinical work of all the care-givers within the practice and collaborates with the Practice Manager and RN manager to ensure staffing supports the clinical needs of the patients. In collaboration with the Director of Primary Care, Regional Medical Director and Primary Care administration, the Medical Director is responsible for meeting quality, cost, efficiency, outcome, and patient engagement metrics for the practice, as assigned by the Director of Primary Care. The Medical Director will have a meaningful understanding of, commitment to, and provide leadership around, patient centeredness and team based care. Also, s/he collaborates with the Department of Medicine residency program and Harvard Medical School to ensure that teaching goals and requirements are met for any trainees in the practice.
The Regional Medical Director
This is a minimum of ½ time (0.5 FTE) for clinical + 1 session as Medical Director for a total minimum 0.625 FTE. Additional clinical time is available.
Regional Medical Director, Regional Administrator, Practice Manager, and all training resources
(coaching, leadership development, strategic planning, team development, etc.) recommended and provided by Primary Care Central Administration. Also, has access to Primary Care Central staff for various leadership, administrative, operational, recruitment, and policy resources.
Duties and Responsibilities:
- Collaboratively hire physicians and advanced practice clinicians for the practice. Responsible for periodic performance evaluations of clinicians within the practice
- Collaboratively creates a team environment with input and skills of each member of the practice.
- Assists the Practice Manager with the hiring and evaluation of the non-provider staff.
- Provides resources to advance the knowledge and practice of team based care.
- Participates in and supports the maintenance of processes to ensure that continuous quality improvement is a core practice competency.
- Provides mentoring and support to faculty as needed.
- Participates in primary care leadership meetings.
- Participates in various committees as requested by the Director of Primary Care.
- Spanish language a plus
