Responsibilities:

Collaborates with consumers to assess their conditions and needs, developing, and supervising personalized care plans.

Conducts comprehensive nursing assessments to gather physical and behavioral health information.

Advocates for consumers and their families, linking them to resources and empowering them to improve their overall health and wellness.

Utilizes motivational interviewing skills to empower consumers towards better health outcomes.

Leads interdisciplinary treatment team meetings to review assessments, updates care plans, and determines follow-up frequency.

Maintains positive working relationships with primary care providers and social service agencies, collaborating to obtain resources and meet consumer needs.

Current and active license to practice as a Registered Nurse.

Minimum of one (1) year experience in a mental health related field or healthcare setting preferred.

Proficiency in MS Word processing software and intermediate general computer skills.

Excellent written and oral communication skills.

Access to reliable transportation and ability to travel locally.

CPR, First Aid, and Therapeutic Options certifications, or ability to obtain within 30 days of employment.

TB test within 12 months prior to employment or ability to obtain within 30 days of employment.
