AJ Home Health, LLC is currently seeking a compassionate and dedicated Registered Nurse to join our team in providing exceptional care to our clients in the comfort of their homes. As a Registered Nurse in our home health division, you will play a crucial role in assisting patients to maintain their independence and achieve optimal health outcomes. Your responsibilities will include conducting assessments, developing personalized care plans, and delivering skilled nursing services to improve overall well-being within the regulatory framework of California's healthcare system.
Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
- Provide skilled nursing care, including wound care, medication administration, and patient education.
- Monitor patients' progress and respond to changes in their condition promptly.
- Coordinate care with other healthcare professionals and community resources to ensure continuity of care.
- Document all patient care activities accurately and timely in accordance with regulatory standards and agency policies.
- Educate patients and their families on disease management, medication management, and self-care techniques.
Requirements:
- Current unrestricted Registered Nurse license in the state of California.
- Minimum of one year of experience as a Registered Nurse, preferably in home health or a related field.
- BLS certification required; additional certifications such as ACLS and PALS are preferred.
- Strong assessment and critical thinking skills with the ability to make sound clinical judgments.
- Excellent communication and interpersonal skills.
- Reliable transportation and a valid driver's license.
- Ability to work independently and as part of a multidisciplinary team.
