The Clinical Transformation Director (CTD) reports directly to the Vice President of Medical Operation and Informatics. The CTD is in charge of relationship building among a diverse group of stakeholders including physicians and other healthcare team members. They will lead change management initiatives across Beebe aligned practices. They will work with the Delmarva Health Network, eBright ACO, and all of our population health programs by providing clinical oversight and strategic leadership advancing our Population Health initiatives throughout our provider community. They will help to implement innovative, value-driven approaches to improving health care to our community. They will help providers make changes as necessary demonstrated through score cards and other monitoring on the ability of our Beebe aligned practices to implement practice transformation initiatives and affect these value driven approaches.
MD or DO. Eligible to be licensed in Delaware and on the Medical Staff at Beebe Healthcare. Has been working as a physician in clinical medicine for at least 5 years. Is willing to continue working some clinical hours. Experience working in a Medicare Shared Savings (MSSP) ACO , Patient Centered Medical Home, Bundled Payment Programs or other advanced payment models. Has experience working in Population Health and has worked in both an acute and ambulatory settings. A skillful, well-spoken individual who partners with physicians and relates to people at all levels of an organization. An energetic and strategic visionary with sound Information Technology skills. A CTD must understand and excel at change management.
The CTD will have clinical time in one of our Population Health clinic programs. The amount of time may vary as needed. The CTD will have clinical time in either an outpatient setting or as a hospitalists at Beebe Healthcare. The amount of time may vary as needed.
The CTD will work with our Post-Acute Care Network and their medical directors and providers in helping to establish care pathways, protocols, and improved communications to further our Population Health strategies.
The CTD will give direct clinical oversight to any of our population health clinics. They will give insight into policy and workflows and ensure that our clinics and strategies are understood and utilized by our aligned physicians. The CTD will work with our Care Coordinators giving clinical oversight, instruction, training and helping the team align strategies across our population health initiatives.
The CTD will be a member of the Delmarva Health Network (DHN) board. They will also be on the Quality Committee. They will work with the DHN Coordinator or Manager on change management across the DHN. They are responsible to assist in improving the clinical outcomes and savings across the network. This is for both MSSP, or Bundled Payment initiatives or other initiatives as the network develops them.
The CTD will work with the CMO in developing and implementing an alignment strategy with our physicians in our primary service area as well as those in our secondary service area.
The CTD will identify clinical initiatives for changing the way care delivery is implemented at the system and practice level to improve outcomes and reducing costs. They will report to individual physicians, practices, DHN board, and Beebe Healthcare key stakeholders the performance of those initiatives. They will help individual physicians or practices to improve their performance when possible. The CTD will work with the CMO on eBright Health initiatives and on any MSSP initiatives to ensure that our MSSP program is a success.
