Director of Care Coordination
- Regularly visit clients and caregivers to ensure the caregiver is fully supported and the clients are prospering and receiving the highest quality of care· Coordinate care for clients. Working with referral partners, ensure clients are getting any needed assistance from skilled nursing, hospice, primary care physicians, financial planners, elder law attorneys and other professional resources. Help clients transition effectively from home to independent, assisted living and memory care communities, nursing homes and from rehab facilities and hospitals. · Seek out and develop strong relationships with like-minded referral partners to coordinate the provision of extraordinary care· Actively seek for opportunities to assist local facilities, communities and organizations with activities and other initiatives to assist seniors and adults in need.· Develop weekly content for web site and social media to educate and keep the communities served up to date· Provide weekly in office and monthly on call support
- A determination to provide seniors and adults with the very best of care· An excellent ability to assess situations and determine holistically what a client and family members need. · Excellent relationship building and communication skills (written and oral) · Strong leadership skills and an ability to coach and work effectively with caregivers· Social worker, LPN or RN or significant experience enabling person to achieve the goals of the position.
