Job Overview
As a Case Manager, you will play a crucial role in coordinating patient care and providing support to individuals in need. This position requires a deep understanding of various medical settings and terminology to ensure the best possible outcomes for patients.
Duties
- Collaborate with healthcare providers to create and implement care plans for patients
- Conduct assessments to determine patient needs and develop appropriate interventions
- Monitor patient progress and adjust care plans as necessary
- Coordinate referrals to specialists and other healthcare services
- Advocate for patients to ensure they receive the necessary care and resources
- Maintain accurate and detailed documentation of patient interactions and progress
Skills
- Experience in ICU, Behavioral health, Emergency medicine, Outpatient, Inpatient, or Pediatrics is preferred
- Knowledge of Medicare and Medicaid
- Knowledge of Managed Care
- Knowledge of medical terminology and physiology
- Strong utilization management skills
- Ability to provide quality patient care with compassion and empathy
Job Types: Full-time, Contract
Pay: $35.66 - $45.17 per hour
Expected hours: No less than 40 per week
