California Licensed LVN, LPN or RN for a Remote Based Chronic Care Coordinator position
We are looking for motivated, self-driven, Care Coordinators to provide high quality chronic care management services on behalf of our client. Care Coordinators are specifically trained to work with patients to help manage their chronic conditions and coach healthier lifestyle choices as directed by the comprehensive care plan created by their physician. As we help implement Medicare’s Care Management Services for our clients, we complete our mission, and that results in healthier and more satisfied patients.
Job Goals:
Care Coordinators call each of their patients once a month to cover the following:
Conduct patient assessments
Chart review
Med reconciliation
Referral coordination
Prescription refill requests
Other activities designed to close gaps in care
Chart / document monthly conversations in the practice's EMR
Care Coordinators are also responsible for conducting verbal enrollments to enroll eligible patients in the program.
On occasion, you may also be required to conduct screening calls, schedule appointments and carry out AWV’s if business needs require this.
Job Requirements:
California LVN, LPN or RN License (must be current)
Mandatory Friday, Saturday and Sunday plus two additional days
EMR charting experience (preferred)
Time management/organizational skills
General computer skills, including Email, Word, Excel and Patient/Medical Systems
Excellent listening skills and phone presence
Remote work experience/conducive work environment/high speed Internet
Bilingual English/Spanish
Chronic Care Management experience (preferred)
Geriactric experience (preferred)
Experience conducting Patient Assessments/Wellness Exams (preferred)
Education:
Bachelors /Associate Degree (Preferred)
