As a Registered Nurse (RN) Case Manager you will be caring for a diverse population of clients in their homes. You may do wound care, fill med planners, teach and/educate, and head to toe assessments just to start. You will be working closely with other members of the team to include aides, therapists, and social workers to develop a care plan specific to your client in coordination with the clients physicians with the goal of helping maintain your client where they love to be the most THEIR HOME! You will be seeing multiple clients in a day (its a great time to catch up on a podcast) and will be traveling in a variety of Iowa weather.
