Provide virtual care and remote patient monitoring (RPM) for patients with chronic diseases. As a Care Manager, you will have the opportunity to develop relationships with patients and providers to monitor patients by taking remote vital sign readings via various biometric devices (glucometer, pulse ox, BP, scale) and compliance with personalized care plans (e.g., medication compliance.) This position is remote.
Qualifications
- Familiarity with patient care
- Comfortable with healthcare software
- Licensed as a Medical Assistant with certification in good standing
- Good verbal & socialization skills
- Fluent in Bengali
Payrate
- Starting at $16/Hr - $19.00/hr
