Summary of Position
The Case Manager is primarily responsible for participating in assessment, evaluation, planning, linkages, monitoring, advocacy and treatment of assigned clients. The Case Manager provides an array of clinical and support services to substance abusing women and men, who are homeless or at immanent risk of homelessness. The Case Manager is responsible for providing quality, sufficient and appropriate services and for maintaining high professional standards.
Core Functions/Knowledge Areas
Drug and alcohol treatment principles and clinical practices, standards and best practices.
Community resources - focusing on stable recovery housing for the short- and long-term.
Crisis intervention - Responds to urgent or emergency situations in a way that minimizes trauma to the client and staff members and results in stabilization
PREFERRED: CAADC or CADC, LCDP, or LCSW
REQUIRED: Bachelor's degree in Behavioral Science, Criminal Justice, Human Services, or related field
PREFERRED: Master's degree in related field
REQUIRED: One year drug and alcohol experience; knowledge of community resources focusing on recovery housing for short- and long-term
PREFERRED: Working knowledge of available community resources
