The Acute Patient Educator is primarily responsible for facilitating and performing 1:1 bedside Modality education to acute patients in the hospital setting, with a special focus on patients within their first 90 days of dialysis (i.e. incident patients). Other responsibilities include mentoring, teaching, supporting, and working closely with the Home Ambassadors from each of the acute regions, as well as providing meaningful and regular Modality education to teammates in the region, by coordinating and supporting educational segments in acute program Home Rooms. The Acute Patient Educator is also responsible for facilitating communication and follow up with to ensure that patients have a timely and smooth transition of care
ESSENTIAL DUTIES & RESPONSIBILITIES
Individual Patient Education
Provides modality education to all appropriate patients. Patients will be identified by MD order or hospital contract terms. This includes prevalent patients. Provides necessary education/materials and help identify and overcome barriers.
Engages with patients and teammates using Right Thinking, exemplifying a high level of integrity and compliance, empowering patients through superior education and honoring their choice
Serves as Home Ambassador to acute teammates and manages communication between acute and chronic programs to ensure smooth transition of care
Models DaVita mission and values to all new and current acute teammates
Maintains current clinical knowledge of all Modalities and ensures that education needs are met in accordance with DaVita’s policies, procedures, and guidelines
May participate in the development and ongoing revision of educational materials
