CSS Healthcare Services is looking for a bi-lingual (English-Spanish) LPN for Home Health

Company Overview:

CSS Corp. is an organization that provides Personal Care and Support, Skilled Nursing, and Community-based healthcare services. The agency is licensed by the State of Georgia to provide community based healthcare services for the young, the elderly and those with developmental disabilities. Services are rendered by Direct Support Professionals (DSP), Certified Nursing Assistants (CNA), and Licensed Practical Nurses who are supervised by experienced Registered Nurses and other support professionals.

Position Description:

This position is primarily responsible for providing direct client care, interpreting and monitoring records, writing reports, documenting client activities, assisting with medications, observing client functional levels and reporting to the RN any changes in the consumer's needs and/or condition by performing the following duties.

*Core duties and responsibilities include the following. Other duties may be assigned.
*Assists the RN in carrying out the plan of care,
*Provides skilled Nursing services under the supervision of an RN, in accordance with CSS Healthcare Services policy and Georgia LPN licensing guidelines.
*Prepares clinical and progress notes and record keeping for the clients. Makes supervisory
visits/ assessments/ skill care under the supervision of the RN.
*Provides instructions to the clients on appropriate self-care and self-help techniques.
*Provides a range of motion exercises and ambulation assistance as needed, observes the client for any changes and reports the consumer's condition to the RN.
*Assists with therapeutic activities, wound care and medication administration.
*Assists the RN in performing specialized duties and other duties as required in the care plan.
*Participates fully in Nursing department’s quality assurance activities, including tracking data, trending and evaluating information. Involved fully in the organization wide quality improvement process and provides date when needed to do so.
*Conducts scheduled and unscheduled visits and provides required reports for analysis.
*Provides service change updates in Home Trak from time to time so that information can be current.
*Works in client's home in various conditions, possible exposure to blood and bodily fluids.
*Works flexible schedule to meet patient's needs.

*Communicates relevant policy information to service recipients and Caregivers alike while on site.
*Attends all required training and maintain personal credentials in compliance.
*Participates in meetings and continuing in-service education as required.
*Performs other duties that may be assigned by the Director of Nursing.

*Valid State of Georgia Licensed Practical Nurse (LPN) license
*Valid Driver License with a good driving record.

*Ability to work under stressful conditions of quick decision making.
*Requires hand-eye coordination and manual delivery as necessary.
*Ability to utilize durable medical equipment in the home.

*Experience working with medically fragile pediatric clients

*Must be Bi-lingual (English-Spanish).

*Required and recent skills and experience: Pediactric care, working with fragile individuals and more.

*This position will require frequent direct contact with clients receiving healthcare services, however, not on a daily basis. Therefore, exposure to infectious diseases in the work place or during the normal course of work duties is moderate to high.
