The Adoption Social Worker will work with clients throughout the state of Connecticut who are working with Family & Children’s Agency to complete an adoption home study and/or are seeking post- placement/post-adoption services. Twenty hours of the postion will include work through CT’s
Adoption Assistance Program (AAP), with the additional 15 hours working with prospective adoptive parents being licensed to adopt and post-adoption families approved for services through CT’s Permancy Placement Services Program (PPSP). This position is located in our Branford, CT office.
AAP a confidential, home based, assessment, education, brief counseling and referral service available to adoptive and guardianship families, and to professionals seeking support for their client families. Using an EAP (Employee Assistance Program) model of short term service delivery, the program offers clients the opportunity to consult with a social worker around any concern that is placing stress on the family. Family & Children’s Agency provides AAP services in the greater Bridgeport, greater Waterbury, and greater New Haven areas.
The responsibilities will include but not be limited to:
Conduct short term assessments and case management for families referred to the Adoption Assistance Program.
Connect adoptive families with appropriate resources in their community to target their identified needs.
Provide educational advocacy to ensure that the academic and developmental needs of children are being met.
Conduct monthly parent support groups for adoptive parents.
Attend monthly Adoption Assistance Program statewide meetings
Work with birth parents, adoptive parents, foster parents, and children around issues of permanency in PPSP program.
Complete adoption home studies according to Connecticut child placing regulations
Complete post-placement visits and reports as required by adoption placing program
Provide support to pre-adoptive families during the waiting period and once a child has been placed in the home
Assist with finalization of adoptions in probate court
Submit billing for all services completed
Assist in adoptive parent group education classes
Assist in handling inquiry calls from potential clients seeking information on adoption
Assist in outreach and marketing efforts
Assist in developing post adoption education programs at FCA and in the community.
Maintain detailed progress notes on case activity and collaborative work for all AAP, PPSP, and home study clients in electronic medical record Licensed Master's level social worker in related field with two years of child welfare/adoption experience.
Awareness of the cultural and socioeconomic characteristics of clients served.
Knowledge of infant and child development.
Knowledge of the life cycle of adoption for individuals and families.
Experience conducting psycho-social assessments.
Experience providing individual and family counseling.
Excellent verbal and written communication.

Networking and community relations.
