As a part of Catholic Charities, Outpatient Mental Health Services, the FIRST program provides comprehensive, team-based treatment aimed at improving the mental health and quality of life for individuals who have experienced a first episode of a schizophrenia spectrum disorder by promoting early identification and providing best treatment practices as soon as possible. The FIRST program uses an evidence based model to help newly diagnosed individuals, usually young adults, retain their functionality and productivity. The FIRST Program is located at 1515 W. 29th street in Cleveland.
The primary role of the case manager will include meeting clients in the community and linking them to various resources. Our ideal candidate will be bi-lingual and have reliable transportation. Combination of education and experience normally represented by a Bachelor's Degree and one to two years related professional experience.
Must have competent oral, written and interpersonal communication skills.
Must be well organized, detail oriented and have the ability to maintain confidentiality.
Ability to work with diverse client population, community organizations as well as staff at all levels.
Must be able to form good working and therapeutic relationships.
