Prime Home Health is expanding and seeking compassionate and skilled LVNs working on PRN basis to join our home health teams working with patients in Pleasanton, Poteet, Lytle and surrounding areas.
LVNs will be working closely with our interdisciplinary team to ensure the highest quality of care. This is a rewarding opportunity to make a meaningful difference in the lives of patients during a challenging time.
Responsibilities:

You will be providing direct patient care in accordance with the established plan of care and individualized patient needs, including administering medications, performing treatments, monitoring vital signs, and managing symptoms to promote comfort and alleviate pain.
- Care Coordination: Collaborate with the interdisciplinary team, including physicians, registered nurses, social workers, and chaplains, to develop and implement comprehensive care plans that address the physical, emotional, and spiritual needs of patients and their families.
- Assessments and Documentation: Conduct thorough assessments of patients' condition, document observations, and maintain accurate and up-to-date medical records, including progress notes, medication administration records, and treatment plans.
- Collaborative Communication: Maintain open and effective communication with the interdisciplinary team, patients, families, and caregivers, ensuring the coordination of care, addressing concerns, and facilitating smooth transitions between healthcare settings.
- Emotional Support: Provide emotional support to patients and their families, promoting a caring and compassionate environment that respects individual beliefs, values, and cultural backgrounds.
- Safety and Compliance: Adhere to infection control protocols, maintain a safe environment for patients and staff, and comply with all relevant laws, regulations, and organizational policies and procedures.
- Professional Development: Stay updated with the latest developments in hospice care, attend educational programs, and participate in ongoing professional development activities to enhance knowledge and skills.
Qualifications:

Valid LVN license issued by the State of Texas.
- Minimum of one year of experience as an LVN in an Adult in-home health setting is required.
- Proficiency in using Kinnser EMR system
- Current CPR BLS certification
- Strong clinical skills and knowledge of nursing principles and practices.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and the interdisciplinary team.
- Compassion, empathy, and sensitivity to the physical and emotional needs of patients and their families.
- Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic healthcare environment.
- Valid driver's license and reliable transportation for home visits (if required).
- Commitment to maintaining confidentiality and upholding ethical standards in patient care.
