As a Senior Quality Improvement Facilitator at Telligen, you will support communities to improve care coordination and transitions of care across healthcare settings through community-level interventions, assessment of provider performance data, identification of gaps in best practices and recommended improvement methods to improve patient care processes and quality outcomes.
You will recruit, develop, and facilitate communities of practice among participating providers, families and beneficiaries and will work one-on-one with identified participants to perform root cause analysis, implement QI strategies, and assist with related systems improvement.
You will assess transitions of care gaps within healthcare communities and recommend improvement strategies and processes to enhance beneficiary outcomes. You will assist in the implementation of the methods and processes within the community.
You will utilize primary and secondary data and information with community providers to identify gaps and assess effectiveness of intervention efforts.
You will partner with team members and internal/external customers as necessary, to help determine, evaluate methods, and mentor to encourage participation in the quality initiatives. This may include research of interventions, data analysis, formal and informal education or presentations.
What you need to make the cut:
Current and valid MPH (Masters of Public Health), RN, LPN, RHIA, RHIT, or MSW or bachelor’s degree with experience in the following areas: community health, public health, or community development.
5 years of experience working in direct collaboration with providers, community organizing, population health or public health, and/or health care quality improvement.
Substance abuse experience, along with mental health or pain management background is highly desired.
Project Management is preferred.
Functional PC knowledge.
Experience with quality improvement initiatives preferred.
Experience with project management or case review preferred.
Ability to travel up to 50% locally or overnight.
