Qualifications:
- Current Texas State Nursing License (LVN/RN).
- Minimum of 1 year of clinical nursing experience preferred.
- Proficient in English, with strong communication and interpersonal skills.
- Demonstrated ability to work effectively in a team and collaborate with interdisciplinary teams.
- Knowledge of evidence-based practices, quality improvement processes, and patient safety protocols.
- Familiarity with Medicare and Medicaid processes is a plus
Responsibilities:
- Collaborate with physicians and the healthcare team to develop and implement personalized care plans, ensuring a holistic approach to resident health.
- Perform comprehensive assessments and monitor residents' conditions.
- Administer medications and treatments accurately, maintaining compliance with all safety protocols and regulations.
- Foster a culture of teamwork and open communication, working closely with nurses, physicians, and other healthcare professionals to ensure continuity of care.
- Educate residents and their families on health conditions, treatment options, and preventive care, promoting informed and engaged decision-making.
- Maintain accurate and thorough documentation in electronic health records (EHR), ensuring compliance with regulatory standards and supporting seamless information flow.
