Position Summary: Assist in providing and directing the health care services for Assisted Living and Memory Care residents across properties in the portfolio. Assist and direct the nurses as needed, provide personal care services, assistance, support, evaluation, and supervision to residents.

Job Responsibilities:
-
Oversee all assigned communities.
-
Maintain a visible presence in the communities working alongside DONs and ADONs to ensure the nursing department is adhering to rules and regulations along with policies and procedures.

Assist with training of new DONs and ADONs.
-
When acting as interim DON, will assume call to assist and maintain communication with Executive Director of community.
-
Have knowledge and understanding of rules and regulations; assist with state surveys.
-
Assist in writing the Plan of Correction with the Community Director for Corporate and State reports.
-
When acting as DON, will assume call to assist.
-
When acting as DON, maintain communication to ED of community.
-
Positive working relationships with teams in each community.
-
Coordinate services to meet the health needs of the residents and promote wellness such as vendor relationships, expectations, and other contracted services.
-
Assist with pre-admission assessments of potential new residents as requested.
-
Work with team to develop and update service plans for residents as requested.
-
Assist in the coordination of the plan of care for at-risk residents.
-
Evaluate any LOC concerns and work with the community’s DON and ED to determine continued eligibility or discharge.
-
Assist DON with corporate regulatory evaluation preparation.
-
Review assessments, resident/family teaching, service plans, ADL logs, MARs, etc. within your assigned communities.

Review the implementation of the resident service plan and evaluation of that plan to include updates for significant changes.

Review resident records according to state and federal guidelines.

When acting as Interim DON, complete job responsibilities of Director of Nursing and report to the Executive Director of the Community.

Report any failure to comply with policy or regulation to the Regional Executive Director and Director of Health Services.

Identify supplies needed to perform tasks and report need to Regional Executive Director and Director of Health Services.

Assist with all investigations. Report to Regional Executive Director upon beginning and completion.

Ensure the security of clinical records.

Work with the management team to ensure efficient operation of communities

Carry out other duties as assigned by supervisor or as necessary to maintain quality and continuity of services and safety of the residents.

Assist in maintaining a safe, home-like, and pleasant environment for residents.

Create and maintain an environment for residents, families, and guests that is customer-friendly and that values and respects the uniqueness of each individual.

Performs all responsibilities under the nursing code of ethics and the nurse practice act.
