Under the direction of a Nurse Manager, performs the delivery of Licensed Home Care Service Agency (LHCSA) PACE participants care as appropriate to provide skilled services in accordance with Agency policies. Preforms delegated nursing skilled responsibilities as per the members plan of care as developed by the Community Health Nurse who is the interdisciplinary (IDT) Registered Nurse assigned to the member.


Provides nursing care under the supervision of a Registered Nurse and according to the plan of care derived from physician's orders and Registered Nurse assessment. Nursing Care includes physical care, emotional support, and PACE participants and family education.

Provides nursing care and services is in accordance with established plan of care and agency policies.
Assists the physician and/or Community Health Nurse (CHN) in the performance of specific procedures required by the PACE participant at home and at an appropriate level for the LPN.
Adheres to strict aseptic technique when performing treatments requiring its use.
Provides PACE participants with the knowledge and assistance to learn appropriate self-care techniques.
Accurately records PACE participant’s condition in the electronic health record of the member.
Observes, records and reports to the appropriate professional the symptoms, reaction and changes including:
General physical and mental condition of PACE participant’s, and signs and symptoms which may be indicative of untoward changes; and
Stresses in human relationships between PACE participants, between PACE participants and personnel, and between PACE participants and their families and visitors.
Assists with the rehabilitation of PACE participants according to the medical care plan by being aware of and encouraging the interests and special needs of PACE participant; encouraging PACE participants to help themselves within their own capabilities in performing activities of daily living; knowledge and application of the principles of prevention of deformities, the normal range of motion, body mechanics, and body alignment.
Promotes cooperative effort among personnel by understanding the functions of the PACE interdisciplinary team (IDT) members involved in PACE participants care and by active participation in team and staff conferences when appropriate.
Recognizes and reports symptoms to the Nurse Manager and/or IDT Manager that indicates changes in the condition of the PACE participant. Communicates to the CHN and/or Nurse Manager any changes in member condition that is necessary for the CHN to conduct a comprehensive RN reassessment for a potential revision to the member’s plan of care.
Administers medications and treatments that are assigned by the Registered Nurse and/or Nurse Manager listed on the PACE participant’s home care plan of care, and records these with accuracy and completeness. Perform medication pill box pre-fill as per established plan of care.
Provides PACE participants and/or family education as required or directed by the registered nurse.
Illicit PACE participants satisfaction to home care services received and communicate to the Nurse Manager and/or CHN any PACE participants dissatisfaction that will result to a grievance/issue investigation
Participates in grievance investigations and other incidence investigation mandated by the PACE program or agency policies
Maintains the PACE participant’s equipment and home environment to ensure safety.
Participates in staff in-service programs. Maintains a continuing knowledge of competencies related to the nursing profession by participation in in-service and educational programs, conferences, workshops and professional organizations.
Demonstrate proficiency in periodic performance/field competency evaluations conducted by the Nurse Manager or designee.
Abides by administrative policies regarding nursing coverage and responsibilities, scheduling availability, and provides support to the PACE Nursing Department as deemed appropriate.
When necessary, works on additional projects within scope of knowledge/practice, as requested by Nurse Manager/Department Director

Computer/laptop proficiency required
Good organizational, observation, and judgment skills.
English proficient in oral and written communication skills required. Bilingual preferred to meet language needs of site membership as applicable
Is self-directed with ability to work with supervision.
Is flexible and cooperative in fulfilling role obligations.
Ability to execute Exceptional Customer Service in a fast-paced PACE center environment
The ability to concentrate on moderate details (multitask) and adapt to frequent interruptions and changing situations. The ability to attend to tasks lasting from ten (10) to ninety (90) minutes.
Must be able to comprehend and relate to theories behind a number of medical concepts.
The ability to transfer, lift, turn or assist a resident to or from beds, wheelchairs, stretchers, mechanical lifts, toilets and showers.
The ability to push or pull residents in wheelchairs or stretchers with a total of 250 pounds unassisted

Works outside in varied weather conditions in all areas of the community, using private or public means of transportation.
Walks to and from PACE participants’ homes.
Potential exposure to health hazards and/or home living situation of the PACE participants.
Works inside in well lighted, heated or air conditioned office and in varied conditions in PACE participants’' homes.

Current NY State unrestricted License as a Licensed Professional Nurse (LPN)
Graduate of an approved LPN/LVN school of nursing
Completed complete NYSED approved infection control coursework. Completed a minimum of 8.0 education contact hours that may include contact hours of mandatory training requirements such as infection control coursework
Minimum one (1) year experience working with the elderly
Minimum of one (1) year in-PACE participants care experience and/or (1) year home care experience preferred
Two (2) satisfactory references from past employers.
Demonstration of satisfactory knowledge and competency for the delivery of services, treatments, and teaching under the direction of a Registered Nurse.
Possess and maintain good physical stamina and mental health, including satisfactory health clearance as required by NYS regulations and Agency policy.
Current Driver’s License, insurance and car available for work is required.
