LSI is seeking a dedicated, compassionate full-time Care Coordinator for our Family, Safety, Risk and Permanency services in the Burlington area. The person in this role is dedicated to serving families in Des Moines, Henry and Lee counties, providing them with the encouragement and support they need to thrive. This role is crucial to keeping kids safe and building stronger Iowa families.

DUTIES

Our Care Coordinator provide direct service to kids and parents by promoting safety and permanency, with a mission to keep our families together. In addition, this role is responsible for:

  • Family Support
  • Crisis Management
  • Case coordination through the use of assessments
  • Developing treatment plans
  • Evaluating family progress
  • Facilitating and monitoring supervised family interactions
  • Providing transportation for families, as needed
Travel is required for this role, as families are seen in their homes, an office, or another community setting.

QUALIFICATIONS

Minimum Qualifications: Associates degree with a major in social work, sociology, psychology, human services, criminal justice, counseling, family therapy, family services, family studies, child & family services, human relations, chemical dependency counselor, nursing, special education, elementary education, secondary education, law enforcement administration, or political science. Major of study must match with what is listed on college transcript-any degree major not listed does not meet requirements. Four years of full-time experience in human services or a related field.

Preferred Qualifications: Bachelor's degree with a major in social work, sociology, psychology, human services, criminal justice, counseling, family therapy, family services, family studies, child & family services, human relations, chemical dependency counselor, nursing, special education, elementary education, secondary education, law enforcement administration, or political science. Major of study must match with what is listed on college transcript-any degree major not listed does not meet requirements. One year of full-time experience in human services or a related field.

This position requires a valid driver's license, auto insurance, a good driving record and the ability to pass an extensive background check including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check.
