Clinical Specialist - Home Visiting

Preventation Services

Provide on-going case consultation on mental health issues to home visitors and supervisors.
Provide training to program staff on mental health and related issues.
Assist program home visitors by joining them as appropriate on home visits to address family-specific mental health concerns.
Role model for appropriate parent/child interaction.
Review and discuss high scores on mental health screens with home visitors and supervisors.
Work with families to identify the need for mental health referrals, as appropriate.
Provide follow-up on referrals made with home visitors and families.
Document all referrals using electronic (Participant Data Systems), paper records, and tracking tool(s).
Maintain and document ongoing communications with service providers.
Participate in service coordination meetings as identified.
Assist identified families in developing and maintaining a natural support network by identifying and utilizing community resources and services.
Educate the community regarding the purpose, function, limits and philosophy of the program during the course of service delivery.
Schedule: Full-time, M-F

Pay rate: $42,000 annual salary

Master’s degree in Counseling, Psychology, Social Work or related field,
licensed or license-eligible for LICSW, LCSW, LMHC, or LMFT with a minimum of three years
experience working with families in a home based environment. Knowledge and experience in
community mental health, child mental health, substance abuse, family violence and related
issues preferred. Good listening skills. Ability to foster self-sufficiency and identify family
strengths in a non-judgmental manner. Ability to work collaboratively with both professional
and non-professionals and have the capacity to work effectively with the service provider
