Per Diem position with On-Call Rotation.
On-Call: On call rotation is one week a month, 7 days including weekends. Must be available for any calls and/ or visits between the hours of 5pm-830am Mon-Fri. and 24hr. availability on Saturday-Sunday.
On-Call Pay: $250 stipend, plus hourly rate if visit is needed.
Service Areas include, but are not limited to: Oak Hills, Victorville, Apple Valley, Hesperia, Barstow, Lucerne Valley
Job Description:
The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Calstro Hospice Inc, either directly or through coordination of care with other spiritual counselors.
Responsibilities-
- Responsible for providing spiritual services that help patients and families to integrate the dying experience into their lives; to find meaning and purpose in what remains of life; and to further their appreciation of spiritual values or sacred values associated with their belief system that give support and hope in coping with the changes taking place.
- Implements spiritual support to ensure that patient and family needs and instructions are effective, current and coordinated with appropriate resources
- Assists with bereavement follow up services as requested/directed
- Assist in developing and implementing the Hospice Plan of Care
- Attends IDG meetings regularly
- Communicates care of the patient with other care providers, using community resources as directed.
- Communicates with other members of the team regarding patient care needs and any change in condition.
- Acts as spiritual liaison between Hospice and the spiritual community.
- Provides non-denominational spiritual counseling per patient and/or family request.
- Completes bereavements assessments prior to and post death, constantly assesses the bereavement risk level of family and plan for the necessary support.
- Keep up with all mailings, communication and groups regarding bereavement.
- Develop and maintain volunteer program, training, orientation and assignments.
- Recruits, educates and selects volunteers from the community.
- On-call weekly rotation for after hour visits if needed.
Qualifications-
- Masters in divinity or equivalent 2 units in clinical pastoral education required
- Hospice Experience preferred
- Emotional and mental maturity.
- Valid state driver’s license and reliable automobile, current automobile insurance and be willing to operate personal car necessitated by nature of job.
- Current health certificate/physical examination and TB testing.
- Maintains patient confidentiality at all times.
- Reports to work on time and as scheduled, completes work within designated time.
- Represents the organization in a positive and professional manner in the community.
- Communicates the mission, ethics, values and goals of the organization.
- Must be willing to commute
