Hiring for Mental Health Service Delivery in Jackson County, Oregon (Medford and Surrounding area)

Provides recovery oriented services to children, adults and families; teaching and mentoring to individuals on caseload. Facilitates and coordinates access to needed services and supports within the community (local schools and with community partners). Implements treatment plan for clients on caseload.

This is a year round position.


Case Manager/Skills Trainer helps formulate and carry out individual treatment plans, working in partnership with other team members. Assists clients with accessing local services and supports such as housing, social security benefits, education, physical health care etc. Also helps teach life skills (as appropriate for each client’s age) such as money management, emotional self-regulation, good interpersonal skills, self-advocacy and utilizing public transportation. The majority of services are delivered in the community. Regularly drives agency vehicle and transports clients as required. Manages time and caseload effectively.


Bachelor’s (BA/BS) degree in a human services field OR three years of experience providing services in a similar setting with individuals who have emotional and/or mental disabilities, OR a combination of education and experience that provides the desired skills, knowledge, and abilities required to perform the job. Related experience preferred.

Bilingual skills a plus; Options pays a premium for employees who can conduct business in Spanish.

Candidates must be able to use e-mail, compose documents, save and locate documents electronically, and learn to use our Electronic Health Record (EHR) system. Proficiency in Word and other MS Office applications preferred. Candidate must be able to work independently and flexibly, under general supervision.

Must pass state-required background and DMV checks; possess a valid OR driver's license and an excellent driving record to transport clients.
