Home Health Care/ Hospice
Addresses the social, emotional and financial needs of patients and their families as these needs relate to their health care needs and concerns, including transition planning. Demonstrates knowledge of process of death, dying and bereavement. Demonstrates the ability to work collaboratively as a member of the Home Health/Hospice Interdisciplinary Team. Demonstrates understanding of the psychosocial factors operative in terminally ill patients and their families. Assists in training of volunteers and staff regarding psychosocial needs of the dying patient/family and bereavement as requested.
Bachelor’s degree in Social Work from a program accredited by the Council of Social Work Education or LBSW required.
Social Work experience in health care settings preferred.
Social Work experience in other practice settings required.
