TITLE: CLIENT RECORDS CLERK
DEPARTMENT: CLIENT RECORDS
FLSA: NON-EXEMPT
GRADE: 2
LINES OF AUTHORITY: This position is supervised by the Client Records Supervisor.
Unless specifically designated, this position does not supervise any other personnel.
POSITION SUMMARY: The main function of this position is to manage the clients Electronic
Medical Record (EMR), including Scanning records received from internal and external sources;
Inventorying physical charts for long term storage and retrieve records in a timely manner;
Processing the release of information requests in a timely manner while adhering all laws and regulations including HIPAA. Maintains client confidentiality.
PRINCIPAL DUTIES/RESPONSIBILITIES:
1. Verifying that scanning of client information is in proper order according to the current filing format.
2. Scanning records, correspondence and miscellaneous forms received from an external source in clients EMR.
3. Sorting incoming faxes
4. Completing after-hours crisis reports according to specified procedure, including forwarding messages to the appropriate staff member
5. Requesting and pulling clients’ physical records from storage to ensure all information is scanned and available to staff.
6. Routing and managing of client records according to policy
7. Performs the function of Release of Information as needed.
8. The employee must work well under pressure, meeting multiple and sometimes conflicting deadlines. The employee shall, at all times, demonstrate cooperative behavior with colleagues and supervisors.
9. Works a flexible schedule as needed to provide coverage at all sites.
10. Participates in the Performance Improvement Processes in the Clinic.
11. Able to effectively de-escalate clients using techniques as trained and outlined in approved crisis intervention models.
12. Other duties as assigned.

POSITION REQUISITES:
Qualifications and Experience
Required:
- High School Diploma or General Equivalency Diploma (“GED”)
- Must have a valid Arizona driver’s license
- Computer experience, office machines, copy machine and facsimile machine
- Experience with medical records or closely related tasks and requirements
Preferred:
- One year experience working in the medical field
- Knowledge of Electronic Medical Records (EMR)
- Microsoft Office software experience
- Knowledge of confidentiality laws, HIPAA

WORKING CONDITIONS:
Physical Requirements:
- Able to sit, stand or walk for extended periods of time.
- Lifting and reaching.
- Able to lift up to 40 lbs.
- Able to walk up and down 2 flights of steps
- Able to drive a vehicle
Equipment Operation:
Computer, scanner, laser printer, facsimile machine, copy machine, shredder and telephone and the ability to drive a vehicle to off-site clinics.
Environmental Conditions:
Air conditioned and/or heated office setting

Job Type: Full-time
Pay: $14.85 - $15.50 per hour
