Job responsibilities:
- Complete an initial assessment of patient and family to determine home care needs
- Initiate the plan of care and makes necessary revisions as patient status and needs change
- Document patient history, symptoms, assessments, tests, and care according to guidelines
- Be a care Manager for your patient
- Educate patients and their families on proper home health care strategies and procedures
Flexible work schedules including after-hours and weekends.
Requirements:
- Must possess a current, valid RN license and Registration
- CPR certified
- Excellent communication skills
- Demonstrated ability to maintain a high level of professionalism during stressful times
- Strong critical thinking skills
- Dedicated to providing the best patient care possible
