General Statement of Duties: Care Coordinator is responsible for face to face assessment, care coordination assistance, health condition coaching and support to individuals transferring to a community setting from a healthcare facility. The Care Coordinator will act as a link between the client and their healthcare team to assist the client with self-management tools for their own care. The Care Coordinator coordinates with other NWICA programs, healthcare entities, hospital and medical personnel to help individuals smoothly transition from healthcare settings with the goal of preventing re-hospitalization.

Qualifications: Bachelor's degree in Sociology, Psychology, Social Work, Nursing, or Gerontology, with at least two years of experience in a human services setting utilizing care plans and service delivery, current certification as Division of Aging Case Manager. Transferable experience in service management, quality improvement, and project/service development. Certified Community Health Workers will be considered.

WEATHERIZATION PROGRAM SUPERVISOR

General Statement of Duties: Reporting to the Community Services Manager, the Weatherization (Wx) Supervisor is responsible for overall operation and performance of the Wx team including staff and contractor performance. This includes performance measurement and reporting, staff performance and development, continuing staff education, community and provider relations, staff relations, meeting performance standards for the service and agency established in contracts with IHCDA and other funding sources, program review and quality oversight for compliance with the State and Federal guidelines and practices.

The Wx Supervisor is expected to be an active participant in the agency's mission, vision, and values. He/she shall work to ensure consistent, quality services to the agency's constituents demonstrate enthusiasm, initiative, and flexibility in assignments and maintain positive working relationships and communication with all agency personnel, will be inclusive, communicative and will establish himself/herself as a primary resource to those he/she supervises.

Qualifications:
  • Associates degree in technical/Engineering, Public Administration or related field and/or three years construction trade experience preferred
  • At least two or more years of supervisory experience.

  • Excellent writing and effective communication skills with staff, clients, service providers and public.

ADRC TECHNICIAN

General Statement of Duties: Position is responsible for overall administrative support of the ADRC and HCBS programs, and Pre-Admission Screening. This includes documentation preparation and processing, data entry and management, monthly activity reporting, customer and community partner support and adherence to funding source guidelines.

Ability to organize and prioritize multiple tasks and manage time efficiently. Ability to operate a fax machine, copier, telephone, adding machine, scanner and computer workstation. Typing 60 WPM. Use of word processing, spreadsheet, and agency database systems.

LTC OMBUDSMAN
(Porter County)

General Statement of Duties: Responsible for providing persons residing in long term care facilities and their representatives with information about their rights and available resources such as community services available and laws, regulations and standards that govern long term care facilities. They will also be charged to empower persons residing in long term care facilities and their representatives to advocate for their rights and they will advocate for residents who cannot be empowered. They will conduct visits with residents living in long term care facilities on an on-going basis to address complaints and to assure that residents receive fair treatment.

Qualifications: Must possess a Bachelor's Degree in Public Administration or related field or 4 years' experience in long term care. Cannot be designated if they've worked in a long term care setting in the past 365 days. Must be free of conflicts as defined by the LTC Ombudsman authorization. Effective communication skills with staff, clients, service providers and general public and have an understanding of low-income client populations are also a must. Reliable transportation is a requirement as well.

General Statement of Duties: Actively participates as a member of the 211 Information and Assistance team; specializes in Medicare, Medicaid and other insurance benefits and provides education, counseling, assistance and advocacy. Attend outreach functions throughout Northwest Indiana Community Action Corporation's six county service areas. Completes special projects as assigned by the Benefits Counseling Supervisor and is responsible for keeping complete and accurate documentation.

Qualifications: Candidates who are fluent in English and Spanish are encouraged to apply. Candidates must have a high school diploma. Bachelor’s Degree preferred. BREASTFEEDING ASSISTANT COORDINATOR

General Statement of Duties: The WIC Breastfeeding Assistant Coordinator will oversee the inventory of pumps and supplies in all WIC Clinics, and Administration office. He/she will provide promotion of breastfeeding in the community, amongst participants and with the WIC Program staff. The WIC Breastfeeding Assistant Coordinator will share information about the WIC Program and its benefits to participants, and assist with WIC Program promotion/education and outreach for the purpose of maintaining and increasing participant utilization.

Qualifications: Candidate should be a qualified Nutritionist, Registered Dietitian, or Licensed Registered Nurse. A Bachelor’s Degree in Family and Consumer Sciences (Home Economics) with an emphasis in Nutrition defined as a minimum of 9 hours in nutrition which include, 3 hours of introductory nutrition, 3 hours in life cycle nutrition, 3 hours in advance nutrition. Candidate should have experience in BF promotion, and have at least 1 year of counseling BF women. BF credentials are a plus (CLS or IBCLC). Candidate should also possess excellent writing/editing and verbal communication skills. Be a self-starter, able to work independently, service oriented with attention to detail. Be committed to work collaboratively with all constituent groups, including staff, volunteers, program participants, and other WIC Program supporters.

Hospital CPA will provide In-services to hospital staff members about breastfeeding, formula, and general WIC information. BREASTFEEDING PEER COUNSELOR

General Statement of Duties:The primary function of the WIC Breastfeeding Peer Counselor is to promote breastfeeding within the community and the clinic in conjunction with outreach activities of the WIC Program. The Peer Counselor recruits and counsels WIC prenatal and nursing mothers as well as provides breastfeeding support and education for new mothers.

Qualifications: Candidate should be a current or previous breastfeeding mother with a minimum of 6 months of breastfeeding experience. Willing to successfully complete WIC Peer Counselor training.
