Overview of the Role:
Alignment Health is seeking a remote LVN or RN nurse health specialist (must be licensed in California) for a long-term temporary engagement (with medical benefits) to provide triaging service for patients who call into the Alignment Health virtual care center. The ideal nurse health specialist will have intensive care unit (ICU), emergency room (ER), and / or medical-surgical (Med-Surg) experience. As a nurse health specialist, you will escalate patient calls to advanced practice clinicians (APC) and manage patient care and treatment in collaboration with the physician and nurse practitioner (NP) / physician assistant (PA). Work schedule: Sunday, Monday, Wednesday, Thursday
- 7:00pm - 6:00am Pacific Time (1 hour lunch)
- 8:00pm – 7:00am Mountain Time (1 hour lunch)
- 9:00pm – 8:00am Central Time (1 hour lunch)
- 10:00pm – 9:00am Eastern Time (1 hour lunch)
- Must have flexible availability (days, evenings, overnight, weekends, and holidays coverage on rotation with advance notice) since team is operates 24 hours a day, 7 days a week.
Responsibilities:
- Answer all inbound calls to the virtual care center.
- Use clinical judgement to address patient concerns.
- Collaborate with primary care physician (PCP), extensivists, and NP / PA, and case manager to develop care plans for members for non-Care Anywhere patients.
- Conduct outbound calls and virtual visits to complete patient follow-up.
- Perform daily review of vitals for patients enrolled in remote patient monitoring program.
- Support disease management referrals.
- Interpret and evaluate diagnostic tests to identify and assess patients’ clinical problems and health care needs.
- Educate members on topics such as disease process, end-of-life, medication, and compliance.
- Discuss cases with physician / NP / PA when appropriate.
- Use electronic medical records.
Required Skills & Experience:
- Minimum 1 year LVN or RN experience in intensive care unit (ICU), emergency room (ER), and / or medical-surgical (Med-Surg) required.
- Must have completed of accredited Nursing Program required; BSN preferred.
- Current, unrestricted LVN or RN license in California required.
- Current, unrestricted LVN or RN licensure in Nevada highly preferred.
- Current, unrestricted compact LVN or RN licensure (Arizona, Florida, North Carolina, Texas) highly preferred.
- Must have CPR certification.
- Knowledge of clinical standards of care.
- Experience in gerontology, adult care, highly preferred.
- Experience in palliative / hospice and complex care management, highly preferred.
- Experience in home health including wound care, highly preferred.
- Knowledge of Medicare Managed Care Plans, preferred.
- Excellent administrative, organizational, and verbal skills.
- Effective communication skills with seniors.
- Computer proficient and comfortable with navigating the internet.
- Self-motivated with ability to work independently.
- Detail oriented.
- Dependable and reliable.
- EMR experience is highly preferred.
- Bilingual English and Spanish preferred.
PAY RANGE: $76,100 - $95,000 annually
