Responsibilities

The Executive Director, Care Coordination reports to a Senior Executive at Atlanta Medical Center(AMC). As a member of the hospital’s senior leadership team, defines the operational structure and guidelines related to care coordination, discharge planning and utilization review / management for patients at AMC including inpatients, the emergency room, and hospital outpatient departments. Sets strategy for coordination of hospital-based care continuum services with system care coordination leadership, population health management services, the Transfer Center, outpatient case management and complex care clinic(s).

This position is responsible for defining, developing, and implementing strategic initiatives for all areas of responsibility. Consolidates, recommends final approval, and administers overall business plan and budget. Serves as the spokesperson and is generally recognized as the subject matter expert. Drives results through effective hiring practices, sound management philosophy, and measurement/management of key performance indicators. Leads development of care management protocols, external relationships with the transfer center, and adjudicates conflicts that arise related to external and internal transfers.

The Executive Director will work collaboratively with AMC and System Care Coordination and other clinical leaders as a resource and advisor, proactively analyzing, seeking out opportunities to improve operational, clinical, quality, and patient satisfaction enhancements. The individual in this role will identify opportunities to optimize performance, build internal competencies and create a more rigorous approach to care coordination throughout the enterprise, bringing a strong focus on accountability and appropriate resources. He/she will partner with external organizations to ensure regulatory compliance and best practice efforts.

This position is responsible for the successful growth, financial, clinical quality, human resource, and patient experience for all areas of responsibility. Maintains strong positive relationships with team members, medical staff, and patients. Participates as an active part of WellStar’s senior leadership team in developing and implementing hospital and System strategic plans and initiatives. Serves as an institutional liaison and industry expert for the Medical Staff, financial intermediaries, payors, vendors, and Administration.

Qualifications

Required Minimum Education: Registered Nurse (RN) graduation from an accredited school of Nursing. Master’s Degree required.

Required Minimum Licensure/Certification:
Currently licensed as a Registered Nurse in the State of Georgia

Required Minimum Experience: At least 10 years in a progressive clinical leadership experience with three (3) to five

(5) years of care coordination leadership required. 12-15 years of total experience in a hospital or health system with a proven track record of success. Clinical practice or clinical experience consistent with field of education. Clinical experience in one or more areas of inpatient discharge management, discharge planning, case management, clinical quality, and/or clinical social work.

Required Minimum Skills: Skill in establishing and maintaining effective working relationships with physicians, hospital staff, and hospital/system leadership, with emphasis on effective communication and follow-through. Skills in change management, identifying problems and recommending solutions. Ability to interpret, adapt, develop, and apply guidelines and procedures. Ability to analyze complex clinical scenarios and apply critical thinking. Working

knowledge of healthcare reimbursement, regulations and policies as they pertain to denials/appeals, documentation and coding. Independent, self-directed, with ability to mentor junior colleagues.
