Services include inpatient crisis stabilization and outpatient mental health care, medication management, inpatient detox, outpatient and residential substance use/addiction treatment, targeted case management, integrated primary & behavioral health care, in-home and in-school counseling, prevention and outreach, and a model clubhouse/day program for adults.

Contracted through Kids Central, Inc., The Centers also provides protective supervision and case management services to children and their families regarding issues of abuse, neglect, abandonment, or exploitation. Provide safety management services and case management to ensure that appropriate interventions and services are provided to address caregiver protective capacities while ensuring the child’s safety, permanency and well-being in relation to the identified needs of the family.

Manage and monitor the progress and safety from intake to permanency. Following Early Service Intervention (ESI) and case assignment, the Family Care Manager will become familiar with the case in its entirety by reading through all documentation provided through the staffing. Complete all required court reports and documentation in accordance with the time frames established through Chapter 39, Administrative Codes, CFOPs, Court orders and/or agency policy and contract to include but not limited to case plans, predisposition studies, judicial review report, status updates, and home studies. Attend all court hearings and proceedings.

Maintain all case files and ensures all required documents are obtained and placed in the case record to facilitate timely transfer of cases to other agencies and for internal or external case audits. Facilitate parent and sibling visit in compliance with court orders taking into account the need to facilitate the parent/child bond. For all children, in out of home care, complete a relative search to develop family connections through the use of a diligent search or family tree.

Attendance and participation at all staffing and family meetings ensuring that all critical parties related to the case are notified in advance and invited to participate. Participate with supervisor reviews/consults of all case records as required as required by the agency. Maintain face to face contact with all children on assigned caseload in their residence in accordance with agency policy.

Qualifications

EDUCATION AND/OR EXPERIENCE

Family Care Manager I
  • BA degree in Human Service related discipline required with a minimum of 1 yr of relevant social services experience with children and families preferred. Current Child Protection Professional Certification preferred.
Family Care Manager II
  • Minimum of two (2) years of experience as a Family Care Manager I or Adoptions Case Manager. BA degree in Human Service related discipline required with a minimum of 2yrs of relevant social services experience with children and families preferred. Current Child Protection Professional Certification required.
Family Care Manager III
  • Minimum of two (2) years of experience as a Family Care Manager II or four (4) years of experience as a Family Care Manager. BA degree in Human Service related discipline required with a minimum of 4yrs of relevant social services experience with children and families referred. Current Child Protection Professional Certification required.
CERTIFICATES, LICENSES, REGISTRATIONS
Must show proof of the appropriate automobile insurance limits ($100,000 / $300,000) per Florida Statutes for this position.

The Centers, Inc. is a National Health Service Corps approved site with a Tier-1 level HPSA score. Certain licensed health care providers may earn up to $50,000 toward student loans in exchange for a two-year commitment at an NHSC-approved site through the NHSC Loan Repayment Program (NHSC LRP). Students pursuing primary care careers may be eligible for the Scholarship Program to cover tuition, fees, miscellaneous costs, and monthly stipend.
