The Social Worker collaborates with members of the healthcare team to facilitate the progression of the patient’s treatment and discharge plan. This position incorporates assessment, care coordination, brief supportive counseling and education of the patient/caregiver/healthcare team to reach appropriate goals from hospitalization and post hospitalization care.
Qualifications:
- LCSW or LSW required in the State of NJ is required.
- BA required.
- Masters required.
- Case Management Certification preferred.
- 1 or more years of experience in acute care hospital setting is preferred.
