JOB DESCRIPTION SUMMARY
Responsible for supporting the Intake Supervisor, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and assisting with the insurance verification and authorization processes.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Ensures compliance with all state, federal, and ACHC referral/intake regulatory requirements.
2. Establishes and maintains positive working relationships with current and potential referral sources.
3. Advises callers of Islands Hospice services, admission requirements and procedures.
4. Ensures seamless transition of patients to home care by acting as a liaison between referral sources, patients, family members of patients, payors, physicians, healthcare personnel, home medical equipment providers and company staff to identify patient service and equipment needs, initiate coordination of care, and initiate admission.
5. Follows up to actively work pending referrals. Documents action in a timely manner within the patient’s medical record.
6. Ensures maximum third party reimbursement through monitoring of insurance verification and authorization processes.
7. Provide regular updates on status of patients being admitted, on-hold, or re-admitted.
8. Builds and monitors community and customer perceptions of Islands Hospice as a high quality provider of services.
9. Gathers, collates, and reports referral statistics including key customer referral trends.
10. Maintains comprehensive working knowledge of Islands Hospice contractual relationships and ensures that patients are admitted according to contract provisions.
11. Participates in quality assessment performance improvement teams and activities.
12. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Islands Hospice.

POSITION QUALIFICATIONS
1. High school diploma. Some college preferred.
2. At least three (3) years of experience in an administrative role, within a medical setting.
3. Verifiable computer skills, including data entry.
4. Ability to market aggressively and deal tactfully with customers and the community.
5. Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practices.
6. Demonstrates good communications, negotiation, and public relations skills.
7. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
