Job Summary:
The RN is responsible for delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation. Also directs and guides patient teaching and activities of ancillary personnel while maintaining standards of professional nursing (resource nurse).
Duties/Responsibilities:

Provides safe patient care and a safe environment for patients and others. Assesses, plans, implements, and evaluates individualized care and needs for each patient; documents the nursing care plan. Perceives patient problems and/or complications through assessment skills. Establishes treatment priorities based on medical and nursing diagnosis.

Accountable for administration, observation charting and reporting effects of drugs, intravenous procedures, and other therapeutic and diagnostic measures in a safe and accurate manner. Practices safety by observing State Nurse Practice Act, Code of Ethics for Nurses and hospital policies and procedures. Completes admission profile, patients problem checklist to establish treatment priorities. Initiates a written nursing care plan individualized to the needs of each patient; implement same. Develops short and long term goals under the plan for each patient. Demonstrates flexibility as patient care needs change.

Modifies nursing care plan as required by changing patient needs as determined by evaluation of care administered and established standards of care. Develops discharge plan with the physician, patients family, and other available resources; drafts discharge summary. Able to organize and achieve quality performance level in new routines, procedures, and skills. Able to implement nursing process providing quality care based on assessed needs of patient. Plans and coordinates patient treatment and activities to allow for optimal patient rest. Understands and assumes responsibility for charge position when designated.

Utilizes the nursing process as reflected in the use of admission profile, nursing care plan, problem oriented charting, delivery of nursing care, discharge planning, and evaluation techniques. Demonstrates resource capability through use of other disciplines; ability to deal effectively with complex assignments; and ability to organize and perform patient care routines at a proficient level. Functions within limits of State Practice Act, hospital policies and procedures.

Participates in clinical instruction programs providing learning experiences for staff personnel and student nurses. Responsible for completions of assignments without interference with quality of work. Able to adapt to change in workload or patient assignments without interference in patient care. Able to perform in an accurate and precise manner in crises and emergency situations when time is of the essence. Achieves assignments and patient care load with appropriate time for in-service, unit meetings, and professional conferences. Approaches interpersonal relationships in a manner which avoids antagonism, reduces conflict, and prevents undue patient anxiety.

Gathers and uses factual data for problem solving, patient and/or staff related conflicts. Cooperates with rotation assignment in the interest of the patient. Communicates pertinent information regarding patients at shift report using nursing care plan approach. Participates in staff meetings and patient conference rounds to coordinate management of medical and nursing patient care. Initiates discharge planning function which includes patient and family instruction/teaching, resource referrals, follow-up assignments, and transport arrangements as necessary. Communicates patient information to Nurse Supervisor and Medical Staff.

Documents Patient Admission Profile identifying problems, establishing priorities for treatment and care, initiating a nursing care plan for each patient, and developing short and long term goals. Makes sound judgments based on factual information. Perceives potential problem situations, and intervenes to offset adverse impact. Consults with, and/or advises appropriate personnel of situations requiring follow-up or attention.
- Provides direction and leadership in patient care to auxiliary personnel through delegation of assignments. Initiates assessment of patient needs, nursing care plan, nursing solutions, and appropriate teaching or instruction method for the patients.

Participates in sharing of knowledge and expertise through in-service, staff meetings, assisting in orientation of new personnel. Identifies own learning needs and seeks opportunities for self-growth and career advancement.

Provides proper notification for absence and tardiness, and is punctual at start of shift. Performs and completes assignments, provides follow through on assignments while providing consistent quality and quantity of work.
