Job Description :
The position works to integrate behavioral health services into primary care settings by assisting primary care staff in developing core behavioral health skills necessary for an integrated service delivery. This position provides individual assessment and brief treatment for a wide range of psychological and behavioral health needs using brief therapy.
Performs behavioral health assessments and brief treatments for a wide range of behavioral health needs. (40%)
Coaches primary care providers and clinic staff to develop the core behavioral health skills needed to provide quality integrated behavioral and physical health services in an outpatient primary care setting. (25%)
Utilizes electronic medical records to document clinical data and patient interactions. (10%)
Acts as a liaison, both internally and externally, to professional services to ensure patient care requirements are met. (5%)
Consults with providers to monitor and manage "at risk" and/or "chronic" patients. (5%)
Develops training materials and leads trainings to improve functions of the integrated health team. (5%)
Supports a capacity of self-care with patients and a healthcare partnership between providers and patients. (5%)
Participates fully in the mission of the clinic, contributing positively to staff training, meetings, and other department initiatives. (5%)
Performs other duties as assigned or required.
Master's degree in social work, behavioral health, public health or a related field
2 years relevant experience
PA licensed professional counselor (LPC) or LCSW
