Position Summary:

The Home Again team is looking for a new Director to lead the Operations of our Assisted Living and Memory Care Communities in Cambridge, WI.

This beautiful new community has wonderful residents and a strong community reputation that we are looking to continue!!

The Community Director is responsible for supervising day-to-day operation during shift and on-call in accordance with the State of Wisconsin Regulations and Home Again’s policies and procedures. The Community Director position acts as an advocate, encourages and supports choice, independence and individuality and, provides confidentiality and respect to all residents.

Position Responsibilities and Standards:

  • Create and maintain facility staffing and schedule
  • New resident related items; paperwork; room set up; documentation
  • Create and maintain activity schedule and coordinate performances
  • Maintain all tracking documentation related to facility, regulations, and procedure requirements along with facilitating all related drills and service needs.
  • Delegation of tasks to Resident Assistants
  • Process assessments/admissions/transfers/discharges
  • Assist RN with deliver and supervise resident care and health monitoring which includes monthly weights, blood pressure/pulse oversight; maintenance of oxygen supplies; physician appointment follow up; blood tests/work
  • Assess need for preventive and restorative programs to ensure optimum quality of life
  • Monitor care provided for by resident assistants
  • May be designated in writing to act during any absence of the Administrator
  • Provide oversight and staff teaching
  • New staff interviews and hire paperwork
  • Payroll and time keep processing
  • Accounting related items

Other duties, including but not limited to:

  • Help orient new residents to surroundings
  • Help coordinate services with the resident or family and others involved in the resident service program
  • Communicate with family and responsible parties regarding inquiries about residents, in compliance with facility policy on confidentiality
  • Follow safety practices and standard precautions
  • Assume own responsibility to demonstrate good body mechanics before ambulating, lifting, or transferring residents
  • Use equipment and supplies safely
  • Report any and all changes in resident’s condition to management in a timely manner
  • Document and chart accordingly
  • Treat all residents with dignity and respect; follow resident rights
  • Other duties as assigned


  • Sincere desire to work with the elderly and physically disabled population
  • Confidentiality of all resident and facility information
  • 18 years of age
  • This position requires tact, sensitivity, and professionalism at all times; ability to work closely with professional and non-professional staff members
  • Good communication skills and supervisory skills; ability to prioritize
  • Emotional stability, tact and understanding of residents’ physical, psychological, social and emotional needs
  • Knowledge of the State of Wisconsin Regulations pertaining to facility license
  • Physical ability to lift and transfer residents
  • Ability to remain calm in high pressure situations
