We are an outpatient multi-specialty healthcare organization operating eleven virtual and physical outpatient clinics nationwide and are expanding access to care to other states. We are looking for a Primary Care Nurse Practitioner to offer telemedicine services and help build our broader telehealth practice. This role will also involve delivering Chronic Care Management (CCM) and Collaborative Care Management (CoCM) services. This position has the potential to grow in financial and professional scope, with a path to take on a leadership role within the organization. The role starts with a flexible time commitment that can be customized to you.
Location: Virtual (fully remote via telemedicine)
The Primary Care Nurse Practitioner works with the treatment team to deliver care and build relationships with patients.
Patient Population: Adults and adolescents requiring primary care services, including preventive care, chronic disease management, and health education.
Responsibilities:
- Provide comprehensive primary care services to individual caseload: improve patient health outcomes, medication management, and address comprehensive healthcare needs.
- Deliver Chronic Care Management services, coordinating care for patients with multiple chronic conditions.
- Implement Collaborative Care Management models to integrate behavioral health into primary care.
- Team-based integrated care: coordination with specialists, therapists, and other healthcare professionals to integrate comprehensive healthcare.
- Educate patients on managing their conditions through lifestyle modifications, medication adherence, and self-management techniques.
- Thematic focus on patient experience: optimizing for safety, staff and patient satisfaction, and access to care.
Required Qualifications:
- Board Certified Nurse Practitioner (FNP-BC, ANP-BC, etc.) (Required)
- Active Prescriptive Authority
- Experience in Chronic Care Management and Collaborative Care Management (Preferred)
Preferred Qualifications:
- Active Virginia License
- Active Maryland License
- Private Insurance Paneling
- 5+ years of full-time clinical experience
- Certification in Chronic Care Management or related fields
