Position summary: The Registered Nurse (RN) or Licensed Practical Nurse provides nursing services and oversees the medication system of a multi-disciplinary mental health treatment team. Manages an assigned caseload of patients, providing nursing care and clinical case management, taking responsibility to improve patient outcomes and to manage resources and assure cost effective, clinically effective, and coordinated services.
Knowledge, Skills & Abilities: Working knowledge of psychiatric medications is essential. Understanding of nursing standards, ability to work with members in the community that are not in a clinical setting and provide medication education to members. Understanding of nursing principles, medication storage, HIPAA standards and medical service coordination. Ability to work with members who have mental illness that may require frequent redirection.
Considerable knowledge of a wide range of medical, behavioral, and psychosocial problems and their treatments. Considerable knowledge of medical terminology, disease processes and their treatment as they relate to clinical interventions and appropriate therapies that are based on medical and/or psychiatric diagnoses. Knowledge of the fundamental principles, techniques, and methods of the recovery process and knowledge of the population being served. Knowledge of task analysis, goal writing and the ability to observe behaviors and communicate these observations in writing and verbally, make recommendations for treatment and document progress. Ability to work cooperatively, creatively, and productively as a member of a service delivery team. Teamwork is an essential job function and is evaluated for both conduct and performance issues. Ability and willingness to take on the service assignments of other team members when absences occur and communicate with other team members in a clear, honest, and open manner.
Travel Requirements: Valid Drivers License and insurance required. Travel will vary depending on the daily schedule for this position. All travel is in the community where the members live and work.
