Provide compasionate support and assistance with activities of daily living such as grooming, dressing, bathing, toileting and feeding in a resident focused and dignified manner and in accordance with the resident's plan of care.

Other duties include:
Promote and participate in the continuity and follow through of rehabilitation/restorative programming as outlined by the therapy and/or restorative departments.
Complete daily assignment in thorough and timely manner including additional assigned tasks such as wheel chair washing, passing ice water, etc.
Observe and report changes in resident condition or behavior to team leader in timely manner.
Maintain the confidentiality of resident/staff information as protected by the HIPAA regulations and/or Three Pillars policies and procedures.
Promote and actively participate in communication (both written and verbal) within and between shifts to ensure continuity of quality care for the residents.
Follow all transfer directives as found on the individualized POC. Report any transfer concerns to team leader immediately.
Perform safety rounds at the beginning and end of each shift. Report any resident safety concerns to team leader immediately.
