Enhabit is hiring an Intake Coordinator / Patient Services Coordinator for our home health branch in Rockville, MD. This position will be responsible for scheduling home visits for the field clinicians and will coordinate patient referrals.
LPN strongly preferred.
Responsibilities

Schedule home visits for the field staff each day and assist with the coordination of patient care.

Ensure geographic scheduling to reduce drive time and to enhance staff satisfaction.

Coordinate patient referrals and serve as a resource for referral sources.

Accumulate admissions and intake information.

Communicate with physicians about orders and patient progress information.
Qualifications

Must possess a high school diploma or equivalent.
- Must have at least two years of experience in the healthcare industry and one year experience in home health.
- Must have demonstrated experience in customer service.

Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice.

Must have basic demonstrated technology skills, including operation of a mobile device.
Preferred:

A graduate of an approved school of professional nursing and licensed in the state in which they currently practice is preferred; may be a registered nurse or licensed professional/vocational nurse.

Experience with intake responsibilities and working with a multi-location company is preferred.
