Responsibilities
Completes functions related to physician signed documentation processing and tracking for Home Health and Hospice patients. Responsibilities include, but are not limited to faxing information to physicians, sorting, scanning and attaching physician signed documentation into Electronic Medical Records (EMR) system. Position requires knowledge and utilization of the document management system.

Utilizes document management system. Mails, faxes, or prepares hand delivery of documents requiring a physician signature and complies with physician preference for transmission method.Responsible for data entry associated with Home Health episodes for tracking documentation, orders sent, orders received with signatures, and ensuring accuracy of data entry in accordance with daily and weekly reconciliation procedures.Reports daily any items outside of agency established timely parameters and coordinates with supervisor and care team staff to comply with agency timeline standards.Ensures all documentation is sent to physicians timely, according to agency standards.Secures the necessary physician signatures and clinical staff signatures for the clinical documentation as required by local, state, and federal requirements.Keeps the agency and supervisor informed on outstanding order statuses for the assigned care team.Provides professional customer service to physicians, physician office personnel and other referral sources.Tracks and provides follow-up actions for any outstanding physician signed face-to-face encounter/visit documents per agency standards.Maintains a safe and clean work area and adheres to facility/corporate safety standards.Maintains positive relationships and rapport with co-workers, associates, patients and their family members and community personnel.Adheres to communication and accountability guidelines established by local and regional administration.Adheres to established confidentiality standards as required by Brookdale and HIPAA concerning community and patient information.

Qualifications
Education and ExperienceHigh School Diploma required. A minimum of two years of experience in a related health care office setting. One year home health office experience preferred. Proficiency with computer use and software applications including Microsoft Office and EMR software preferred.

Certifications, Licenses, and Other Special RequirementsAbility to work varying hours, including weekends, nights and holidays.

Management/Decision MakingUses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.

Knowledge and SkillsHas a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has working knowledge of the organization. Must have knowledge and working capability of PCs, including Microsoft Office software. Must have exceptional customer service and organizational skills.
