Responsibilities of the Case Manager include, but will not be limited to:
· Assess, develop and implement Pre-Voc plans using an Interdisciplinary Team approach that will help those individuals he/she is servicing in becoming more integrated with the community regarding, vocational and interactive social skills.
· Ensure compliance regarding agency (ARC}, state (OPWDD), City (Dept. of Mental Health, Mental Health & Alcoholism Services, Dept. of Labor (DOL) and Medicaid • Waiver regulatory guidelines.
· Maintain a Caseload: Approx 25-30 individuals
· Complete and accurately document Community Pre-Voc service plan progress on an Annual, Semi-Annual, Monthly, and/or Daily basis.
· Collect Program Participant Data for the Pre-Voc Plan by utilizing accumulate records, direct observation, staff and other significant input. Daily collected data will help the Case Manager to write better case notes and monthly notes that reflect each individual's needs.
· Conduct two (2) spot checks and two (2) full site visits monthly.
· Provide crisis intervention on an as needed basis and be a team player at all times; maintaining clear communication with management. Be interactive and proactive before a situation becomes reactive.
· Provide crisis intervention and use proper First Aid or CPR techniques when needed.
· Report all incidents and minor occurrences and ensure proper documentation (i.e. case note).

Requirements:
· Minimum High School Diploma/GED Required
· Minimum 3 years experience with the I/DD population
· Valid NYS driver’s license required with at least 3 years of driving record
