Position Title: Early Intervention Service Coordinator

General Description:
The Early Intervention Service Coordinator understands and facilitates the entire Early Intervention Process for eligible children (ages 0-3) from initial referral through transition. Early Intervention Service Coordinators build meaningful relationships with people, families, teams, and community partners while completing essential case management responsibilities for 55-65 children. Service Coordinators are committed to helping those in need and have strong advocacy skills. Must be adaptable to change and have the ability to produce work in a variety of environments and locations while meeting multiple timelines. Must have excellent verbal and written communication skills and the ability to gather information from various sources to develop plans and reports. Requires great documentation skills, comfort with technology, and using Microsoft office programs and electronic files. Must be able to type 40 WPM or more. Requires extensive driving responsibilities.

Job Qualifications:
Education/Experience or Formal Training:
A Bachelor's degree from an accredited college or university; or, a two-year degree plus two years of experience working with families and children with disabilities; or five years’ experience working with families and children with disabilities (experience must be specifically documented and verifiable on your application.) Excellent written and verbal communication skills. Strong advocacy skills.

Smart Phones, Computer, Printer, Fax Machine, Copy Machine, Personal Vehicle, etc.

Working Environment/Physical Activities:
Office and Community Locations. Moderate physical activity required. Frequent sitting and standing, to include sitting on the floor at times. Daily typing. Frequent driving. A working vehicle, valid CO driver’s license (unless active Military family), and minimum insurance levels ($100,000/$300,000/$100,000 liability coverage) are required.

Salary Range: $15.00-$19.00 an hour (depending on education and experience)
Mileage Reimbursement: $.51 per mile or current IRS rates

Training: Early Intervention Program Training provided
